Welcome to the 2010- 11 Middle School Athletic Year.

Attached you will find a calendar which details the practice and game schedules of all of our sports for the month of August, a Physical Form, an Emergency Medical Authorization form and an Insurance Waiver form.

The Middle School offers the following interscholastic sports teams:

There are tryouts and cuts in Volleyball and Boys' and Girls' Basketball

Student-Athletes must have the following:
1. The Middle School Athletic Fee as determined by the Board of Education is $42.50 per athlete per sport.  Please make checks payable to Nordonia Middle School Athletics. The Athletic Fee is non-refundable.  If your family is on free/reduced lunch program a Fee Waiver Form is available in the athletic off.e  This form must be filled out to have the athltic fee waived. The fee or waiver form  is due 5 days before a uniform is passed out. 2. A completed physical examination form (within the past year of the sports season) on file in the athletic office. It is mandatory that an athlete have a physical and questionnaire completely filled in and signed by the first day of practice. Because of the expanded question-and-answer section on family history, student- athletes are to complete the front page of the form with their parents or guardians prior to their physical exam. Also, on the back of the form, please make sure that the examiner includes the date the exam was performed beside his or her signature. Lastly, you are reminded there is a place on the form for parents and students to sign which states that you have read and understand the OHSAA Athletic Eligibility Information Bulletin. The OHSAA Authorization Form must be filled out ONCE for the duration of the student�s enrollment in the school.

5. Academic Eligibility: A student in the 7th grade for the first time will be eligible for the first grading period regardless of previous academic achievement. Thereafter, in order to be eligible, a student in grade 7 or 8 must have received passing grades (D or better) in 75% of those subjects carried the preceding grading period and must have maintained at least a 1.5 grade point average for that grading period.


On Monday, August 25, 2010 there will be a mandatory meeting for the parents of any student interested in participating in sports during the fall, winter or spring sports� seasons. The meeting will be held in the main gym at the middle school and will last from 5:00 PM to 5:30 PM. The meeting shall consist of a review of the student-eligibility bulletin and key essential eligibility requirements; and a review of the school�s Athletic Code of conduct. The meeting will have a sportsmanship, ethics and integrity component and give you the opportunity to meet the coaches at the middle school and will provide you with an overview of the coming year activities of the Nordonia Athletic Boosters.
6. An athlete must reimburse the school district for the replacement cost of lost or damaged equipment from previous sports seasons before he/she will be permitted to begin participation in any other athletic activity.

The purpose of the enclosed calendar is to allow both parents and athletes the opportunity to plan for the start of the Fall Sports Season. There is a key on each page of the calendar designating each sport (for example 8FB = 8th grade football & BSOC is boys' soccer). We recommend you highlight, with a bright marker, the sport you are involved in.

Practice times and contests are also on the Nordonia Middle School Athletic Web Site If there is ever a need to cancel a practice or contest it will be posted immediately on our web site.

There will be no sign-up procedure for fall sports. Student-Athletes wishing to participate, simply attend the first practice, dressed appropriately, with a completed Physical Form, OHSAA Authorization Form, Emergency Medical Authorization Form, Insurance Waiver.

As a member of a Nordonia Middle School Athletic Team, you will be issued a uniform and/or equipment.  You are responsible for the upkeep of this uniform.  The uniform/equipment will be turned in at the end of the season.  You are responsible for the loss or damage of any part of this uniform.  At the end of the season, the coach and athletic director will determine if any costs due to loss, damage, or abnormal wear and tear are to be assessed.

ATHLETIC DEPARTMENT INSURANCE WAIVER

 

 

 

We the undersigned, parent(s)/guardian(s) of ________________________ do hereby release the Nordonia Athletic department from any and all financial responsibilities as a result of any and all injuries incurred by our son/daughter as a direct result of his/her participation in pre-season conditioning programs and interscholastic athletics.

 

We certify that such injuries which might be sustained by our son/daughter as a result of participation in pre-season conditioning programs and interscholastic athletics are covered by our family insurance.

 

For those parents/guardians who want additional coverage, the Nordonia Board of Education encourages you to enroll your student in an insurance program.
